Replacement Credential and Diploma Cover

Replacement Credential Request (Diplomas and Certificates)

If a graduate would like to request a credential reflecting Messiah's new university status---or if a graduate's original credential is misplaced, damaged or a duplicate is needed for employment purposes, they may request a replacement diploma or certificate by submitting a formal signed request to the Registrar's Office in the School of Graduate and Professional Studies. If your credential arrived damaged, please contact the graduate registrar's office at gradregistrar@messiah.edu.

*Please be advised that beginning July 1, 2020, all replacement credentials will be issued under the Messiah University name with the current format, size, and school officials' signatures and names. (Effective July 1, 2020, the dimensions of the Messiah University diploma are 11" x 8.5" landscape).

For previous Messiah College graduates, a notation will be made on the replacement credential stating "Original (diploma/certificate) issued by Messiah College."

1. REQUEST: Please complete the . Type your responses into the fields or use the mouse to select options, then print and sign the form. Adobe® Acrobat® Reader® is required when viewing, completing, and printing PDF files. You can download a free version of this software by .

IMPORTANT: Messiah University requires the penned signature of the student to release academic records. Electronic/digital signatures will not be accepted.

2. PAYMENT: The payment and processing (including mailing) for a new credential is $30. To pay by credit card, please click the Credit Card Payment link on the request form or click on the link provided below.

3. UPLOAD: Scan your signed/completed form and upload it using Messiah's Secure File Upload Service. to upload your request.

You will receive a notification once your file has been uploaded successfully, as well as an email confirmation to the address you provided. Your request will be processed once the Registrar's Office has received your signed/completed form and payment.

 

Replacement Diploma Cover Request
